Stephen Lewis M.D.

Physiatrist | Founder & National Medical Director

Stephen Lewis, MD, is a board-certified physiatrist and founder and national medical director of GamePlan Medical, a multi-state care navigation and medical services organization focused on functional aging, brain health, and whole-person support between primary care visits.

Throughout his career, Dr. Lewis has focused on translating rehabilitation science into scalable, real-world care models that improve daily function, independence, and quality of life. He has led implementation of team-based care workflows—including structured interdisciplinary huddles—across the nation’s largest inpatient rehabilitation system, gaining firsthand experience operationalizing complex, outcomes-driven models at scale.

Dr. Lewis currently leads implementation of the CMS GUIDE Model for dementia care navigation and is advancing upstream, function-focused programs aligned with CMS ACCESS and MAHA ELEVATE priorities, including brain care navigation, balance and fall-risk reduction, and cardiometabolic lifestyle coaching. He is a co-author of the Telehealth Innovation White Paper for the American Academy of Physical Medicine and Rehabilitation and has spent more than a decade developing telehealth-enabled rehabilitation and prevention models spanning cardiac rehabilitation, pulmonary rehabilitation, diabetes prevention, and brain health.

Under his leadership, GamePlan Medical has built AI-enabled care navigation workflows that allow trained community members to deliver consistent, patient-selected support while physicians provide oversight and medical direction. His work positions physiatry as the natural specialty to guide care navigation teams focused on lifestyle, cognitive, social, and functional outcomes—bridging medical guidance with everyday life.
Dr. Lewis is a formal implementation partner with CPESN USA, supporting the scaling of doctor-guided care navigation across a national network of more than 3,500 community pharmacies. He trained at the Shirley Ryan AbilityLab of Northwestern University Feinberg School of Medicine, the nation’s leading rehabilitation hospital, where he developed deep expertise in neurorehabilitation, interdisciplinary care, and function-first models of aging well.

Amina Abubakar, PharmD.

National Pharmacy Implementation & Community Care Navigation Lead

Amina is a nationally recognized pharmacy and community care leader with extensive experience implementing and scaling care management and care navigation programs across independent community pharmacy networks. She brings deep expertise in translating policy, CMS payment models, and clinical goals into operational programs that work in real-world community settings.

She serves as a luminary within CPESN USA, which is a formal partner with GamePlan Medical to support the nationwide deployment of a care navigation workforce aligned with CMS bundled payment and innovation models. Her career has focused on building sustainable, team-based models that allow community organizations to deliver higher-value services without disrupting existing workflows. 

Amina has led multi-site implementations of care management, medication synchronization, and community health programs, with a strong emphasis on workforce training, operational consistency, and measurable outcomes.
At GamePlan Medical, Amina plays a central role in national implementation strategy—helping pharmacies, nonprofits, and community organizations operationalize doctor-guided care navigation aligned with CMS innovation models. Her work ensures that organizations can get paid for what they already do by activating trained community members as care navigators, supported by structured workflows, physician oversight, and AI-enabled decision support. She is instrumental in scaling the CMS GUIDE care navigation workforce model helping tp lay the foundation for emerging upstream CMS initiatives

Swat, PharmD, MS.

National Strategy & Implementation Lead, Care Navigation

Swat is a nationally recognized healthcare strategy and implementation leader with deep experience operationalizing care navigation and value-based service models across community pharmacy and population health networks. He serves as an implementation specialist within the CPESN USA national network and is a key leader in advancing care navigation programs across community-based settings throughout the United States.

His career has focused on translating CMS policy, bundled payment models, and innovation initiatives into executable, scalable operating models that work in real-world environments. Swat has led multi-site implementations across diverse geographies, helping organizations integrate new care navigation workflows into existing operations without disrupting core services. 

His expertise spans workforce activation, standardized program rollout, technology enablement, and performance measurement.
At GamePlan Medical, Swat plays a central role in national implementation and scaling strategy—supporting the deployment of doctor-guided care navigation across community pharmacies, nonprofits, and mobile health organizations. Working closely with CPESN USA and community partners, he helps operationalize CMS-aligned care navigation models, including the CMS GUIDE Model and other emerging upstream models. His work ensures that community organizations can activate trained local staff as care navigators—supported by physician oversight, structured data collection, and AI-enabled decision support—so they can deliver reimbursable, outcomes-driven services at national scale.

David X. Cifu, MD

Senior Advisor, Science & National Implementation

David X. Cifu, MD, is a nationally recognized leader in rehabilitation medicine, brain health, and large-scale care model implementation. A former President of the American Academy of Physical Medicine and Rehabilitation (AAPM&R) and long-time editor of the field’s leading academic textbook, Dr. Cifu has spent his career shaping how rehabilitation science is translated into clinical practice, workforce training, and national systems of care.

Dr. Cifu has held senior leadership roles across academic medicine, the Veterans Health Administration, and national rehabilitation organizations, where he helped design, govern, and scale interdisciplinary models focused on function, cognition, mobility, and quality of life. His work has consistently emphasized preventive rehabilitation—using lifestyle, behavioral, and functional interventions upstream to reduce disability, chronic disease burden, and institutionalization.

At GamePlan Medical, Dr. Cifu serves as Senior Advisor for Science and National Implementation, guiding the clinical and scientific foundations of doctor-guided care navigation models aligned with CMS innovation priorities. He provides strategic oversight on workforce design, training standards, and outcomes measurement for care navigation programs spanning dementia support (GUIDE), whole-person prevention (ACCESS), and upstream functional aging (MAHA ELEVATE). His leadership reinforces physiatry’s role as the natural specialty to oversee care navigation teams focused on functional, cognitive, and behavioral outcomes at national scale.

Aly Cohen, MD, FACR

Senior Advisor, Integrative Care Navigation

Dr. Aly Cohen is a board-certified physician in rheumatology and integrative medicine and a nationally recognized leader in preventive, lifestyle-based, and whole-person care. She brings deep expertise in translating integrative medicine principles into structured, scalable care pathways that support chronic disease management, functional health, and long-term resilience across diverse populations.

Dr. Cohen has built and led national curricula that train integrative medicine physicians throughout the United States, advising on evidence-based clinical guidelines, care pathways, and best practices that integrate lifestyle medicine, environmental health, nutrition, stress management, and behavioral health into routine care. She holds academic and teaching roles through major integrative medicine training programs and serves as a trusted advisor to clinicians and organizations seeking to operationalize whole-person care at scale. 

At GamePlan Medical, Dr. Cohen serves as Senior Advisor for Integrative Medicine, helping embed preventive, lifestyle-focused, and environmental health principles into doctor-guided care navigation models aligned with CMS innovation priorities. Her work supports the development of non-clinical, community-based care navigation pathways that reinforce brain health, cardiometabolic health, functional strength, and behavioral well-being—extending medical care beyond the clinic and into daily life.
