Dr. Lewis is a board-certified Preventative Physiatrist and National Medical Director for GamePlan Medical, where he leads the integration of rehabilitation medicine into value-based dementia care, chronic disease rehabilitation, and home-based recovery models.
He completed his clinical training in Physical Medicine and Rehabilitation at the Shirley Ryan AbilityLab of Northwestern University School of Medicine and holds advanced training in Geriatric Medicine from Mount Sinai School of Medicine and Integrative Medicine from Harvard.
Dr. Lewis has pioneered tech-enabled rehabilitation and dementia prevention programs designed to reduce hospitalizations and improve function in older adults. He is also the creator of HEALTHGAMEPLAN, a first-of-its-kind multi-condition tele-rehabilitation and coaching platform.
He has collaborated with leaders from the American Heart Association, the American College of Sports Medicine, the American Dietetic Association, and the Global Brain Care Coalition to expand access to health coaching, digital transformation, and brain health initiatives. Dr. Lewis also serves as volunteer faculty and Clinical Assistant Professor at Virtua Health College of Medicine.

Drawing on her experience as an award-winning pharmacist and healthcare entrepreneur, Amina leads the development of scalable models for care navigation, integrated pharmacy services, and caregiver support under Medicare’s GUIDE model. Through GamePlan Medical, she empowers healthcare teams to collaborate and deliver patient-centered, measurable outcomes by aligning clinical care and community-based resources.
Dr. Saswat “Swat” Kabisatpathy is a nationally recognized pharmacy leader and data strategist dedicated to advancing value-based care. As the National Director of Strategy and Health Data Analytics at GamePlan Medical, he oversees the design and implementation of data-driven frameworks that guide dementia care and population health initiatives across the U.S.
A Board-Certified Medication Therapy Management Specialist with a background in chemistry research, Dr. Swat brings a unique blend of clinical insight and analytical expertise. His prior leadership roles include Chief Strategic Officer at Avant Pharmacy & Wellness and Director of Strategy and Health Data Analytics at the Avant Institute. His work has shaped innovative approaches to medication management, cost containment, and outcomes measurement across multi-site healthcare organizations. Dr. Swat continues to champion collaboration between pharmacists, primary care teams, and community organizations—turning data into actionable strategies that drive better health outcomes and sustainable value-based models.
